Annex A: Instructions regarding data formats for completing the declaration forms

Declarations under Section J
Section J of the Declarations Handbook applies to the declaration requirements for any facility or establishment under a State Partys ownership or possession, or located in any place under its jurisdiction or control, and that has been designed, constructed or used since 1 January 1946 primarily for the development of chemical weapons. Such facilities shall include, inter alia, laboratories and test and evaluation sites, in accordance with paragraphs 1(d) of Article III of the Convention.

Declaration requirements and their deadlines
In its declaration, the State Party shall specify the precise location, nature and general scope of activities of each declared facility or establishment. The declaration shall be submitted to the Organisation not later than 30 days after the Convention enters into force for the State Party.
The following CWC definition is important when assessing whether a facility or establishment must be declared under Article III(1)(d) of the Convention:
Chemical Weapons means the following, together or separately:
(a) Toxic chemicals and their precursors, except where intended for purposes not prohibited under this Convention, as long as the types and quantities are consistent with such purposes;(b) Munitions and devices, specifically designed to cause death or other harm through the toxic properties of those toxic chemicals specified in subparagraph (a), which would be released as a result of the employment of such munitions and devices;(c) Any equipment specifically designed for use directly in connection with the employment of munitions and devices specified in subparagraph (b).
Instructions in regard to data formats:
1. The page header
The page header should be completed by the State Party submitting the declaration:

2. Yes-No Questions must always be answered.
3. Geographical co-ordinates are to be declared to the nearest second according to the following formats:
Latitude: 00 (degree)/00(minute)/00(second)/0 (North or South)
Example: "55/45/28/N"
Longitude: 000 (degree)/00(minute)/00(second)/0 (East or West)
Example: "005/32/53/E"
Co-ordinates should be associated with a specific feature related to the location (e.g., entrance of a site, road intersection). The geographic reference system used should be specified.
4. A site diagram for each facility is to be attached showing:
(a) a clearly identified declared perimeter. The perimeter will be used during inspections and should indicate, where possible, man-made or natural features (e.g., roads, fences, railroad tracks, or rivers);(b) roads, fences, railroads, rivers, and any other significant geographical features in the vicinity of the facility;(c) the location of all buildings, bunkers, and storage areas within the declared perimeter, with building structure numbers, and an indication of the purpose of each. All buildings, bunkers and storage areas within the declared perimeter that are not related to CW storage should be so annotated. The purpose of each building, bunker and storage area may be included in an attachment to the site diagram;(d) an arrow indicating true north;(e) a legend explaining any abbreviations and a map scale;(f) all entrances/exits including personnel gates to the facility;(g) where the geographic co-ordinates (in paragraph 11 above) were taken.
5. Attachments, if any, are to be identified by unique labels, reference numbers or file names.
6. If declaration information as a whole is to be treated as confidential, this should be indicated on Form OF 1.0 (Identification declaration: Other Facilities), with the appropriate classification code:
R: OPCW Restricted
P: OPCW Protected
H: OPCW Highly Protected

1.1. Form OF 1.0: Identification declaration: Other Facilities is to be used by a State Party:
· to identify the document as a declaration that has been submitted by a State Party pursuant to subparagraph 1(d) of Article III of the Convention;· to provide information about the submitting State Party and the date of the submission;· to assign the level of confidentiality classification of the document.

1.2. Narrative, attachments or any other means can be used to specify the precise location of the facility, providing, at a minimum, the address, the location of the complex, location of the facility within the complex, if applicable, including the specific building and structure number, if any. The location may also be expressed by geographical co-ordinates and a site diagram |

Use the check boxes provided, attachments or any other means to describe the nature of the facility. |
| General scope of activities at the facility: |
Narrative, attachments or any other means can be used to describe the general scope of activities at the facility. The information may be expressed in terms of the length of time (period) the facility was/has been operating, its present status, a diagram showing the layout of the facility, process diagrams, the types of chemicals developed, tested or evaluated, the types of experiments conducted, etc. |
